Western Trust launches new Community Nursing Home and Hub Service

Rapid Response and District Nursing services unite under a new single model starting this June.
Frontline patient care across the West is set for a major transformation as the Western Health & Social Care Trust and merges two of its key community medical teams into a single, streamlined operation.
Starting on Monday, June 1 2026, the District Nursing and Rapid Response services will formally integrate into the newly established Community Nursing Home and Hub Service, a move which will provide a more streamlined and responsive service for the community, ensuring continuity of care and better outcomes for those they support.
Assistant Director of Primary & Community Care, Paula Devine, said: “District nurses and rapid response nurses play a vital role in improving patient outcomes by delivering immediate, specialised care directly where it is needed.
“Together, they help bridge the gap between acute hospitals and local communities, preventing unnecessary hospital admissions, easing pressure on emergency departments and enhancing overall patient safety.
“I am extremely proud of the dedication and hard work shown by staff at every level as we developed this model together. It has been truly inspiring to see everyone collaborating so closely and supporting each other, always keeping the best interests of our patients, families, and carers at the centre of what we do. “Their commitment to improvement is clear as is their unwavering focus on delivering the highest standards of care.”
